No more poker, pure arithmetic
Between roughly 8 and 15 big blinds (a bit higher with antes), the middle ground dies: a raise you'd FOLD to a shove has already burned the raise — that money was a blind, not a bet. First-in play becomes jam-or-fold. The jam range widens with position: under the gun it's narrow, on the button it's enormous — you only need the blinds to fold often enough, and they mostly will. The reference point is the Nash push-fold equilibrium; you deviate for your neighbors: vs a caller who never folds, tighten the jams; vs a big blind that surrenders, widen without shame. And the standing rule: CALLING all-ins needs more equity than JAMMING does — a shove collects folds, a call never does, and ICM taxes both.
